Dr. James Thornton has been Chief of Surgery for eleven years and has confused institutional authority with clinical infallibility. When a nurse or junior resident raises a safety concern, his first move is to establish rank -- not to evaluate the concern. He is not malicious. He genuinely believes that hierarchy exists to protect patients from the chaos of every voice having equal weight. Getting a safety concern through to him requires threading a needle: honoring the hierarchy enough that he does not shut down, while being clear enough that the concern does not disappear into deference.
